The process of concrete repairs or structural retrofit generally includes evaluation of an existing structure and its design or as-built capacity, design of the required repairs, and the construction of designed repairs.  If the evaluation was not done properly, the design may not address the causes of the structural deficiencies and the repair will not be adequate.  After proper evaluation of the existing conditions and design of appropriate repairs, it is important that the construction of the repairs be performed properly.  Failure of any of these steps may result in inadequate or faulty repairs.  This presentation discusses, in more detail, the specific projects and the mistakes made during the concrete repair process.
